      This form is required to be completed by the parent/legal guardian for any child 14 or younger who is participating in either a camp or clinic. This form can be mailed or faxed to the attention of the camp director at the following:

  • Informed Consent Form for children 15 years or older 
     
      This form is required to be completed by the participant (AND parent/legal guardian for any child age 15 to 17 years) who is participating in either a camp or clinic. This form can be mailed or faxed to the attention of the camp director at the following:

  • George Washington Medical Form 
     
      This form is required to be completed by the parent/legal guardian for any child under the age of 18 who is participating in either a camp or clinic that is 3 or more consecutive days. This form can be mailed or faxed to the attention of the camp director at the following:

  • Authorization to Release Child 
     
      This form is required to be completed by the parent/legal guardian for any child under the age of 18 who is participating in either a camp or clinic and will be picked up by someone other than the parent/legal guardian.
